Legacy Family Psychiatry is not contracted with any insurance payers including Medicare and Medicaid. Insurance company models do not adequately allow the extensive amount of time, detail, and flexibility it often takes to complete a comprehensive psychiatric or functional Medicine assessment and discuss options for treatment. Insurance Information
However, your insurance may allow you to see an “out of network” physician and seek “out of network” reimbursement. When requested, we can provide a “super bill”, which is a billing invoice with necessary information for you to submit to your insurance company for potential reimbursement.
Depending on your policy, you may receive up to 75% reimbursement forLegacy Family Psychiatry services. Your insurance company can inform you of the amount they reimburse you for out of network care.
The payment for the visit is due 24 hours before time of service. Payments may be submitted via cash, Visa, Mastercard, American Express, and Discover.

New Patient Evaluation
Bringing a unique patient experience
New patients are required to have an initial psychiatric evaluation, which is a valuable opportunity for the physician to understand the medical, psychological, and social factors involved in making an accurate diagnosis and creating a treatment plan. Plan to spend about an hour for this appointment, which typically lasts 60-75 minutes.
For child or adolescent visits, parents or guardians must attend. Usually, doctors spend 20-30 minutes with parents/guardians, 30-40 minutes with the child/adolescent when appropriate, and, if needed, 10-20 minutes with the whole family to review treatment recommendations. Please provide prior testing, treatment, and school records before or at the evaluation through the patient portal questionnaires. Treatment plans are personalized, based on evidence-based approaches, and may include medication, referrals, lab work, therapy, and suggestions for lifestyle, environmental, or behavioral/parenting changes.
A 40% discount is offered for initial evaluations if referred from another facility where Dr. Swarup treated you within the last year, and these evaluations are often shorter due to prior familiarity.
Follow up / Return Visits

Return visits for medication management usually last about 20–25 minutes. For patients with more complex needs or when psychotherapy is combined with medication management, extended visits of 30 minutes or more are available. How often these visits occur depends on changes in the medication plan and overall psychiatric stability.y.
