Patient Policies

Please review our appointment, cancellation, and insurance guidelines carefully.

Patient Policies

If you are unable to attend your scheduled appointment, we kindly request that you notify our practice via phone or email at least two full business days in advance. All patient policies are also detailed in the new patient questionnaire.

Insurance Policy:

Legacy Family Psychiatry does not accept insurance. We are not contracted with any insurance providers, including Medicare and Medicaid. Patients will be required to pay the fee in full at the time of the visit.

Insurance companies typically do not allow for the comprehensive amount of time, detail, and flexibility required to conduct a thorough psychiatric and medical assessment and discuss treatment options. Our practice philosophy at Legacy Family Psychiatry is to offer more detailed care, improved accessibility, and longer visits than what is generally available in insurance models.

Although we are not in-network with your insurance, your policy may allow you to see our psychiatrists as an "out of network" provider and request "out of network" reimbursement. Your insurance company can inform you about the reimbursement amount for out-of-network care. Upon request, we can provide billing invoices containing the necessary information (“Super Bill”) for you to submit to your insurance company for potential reimbursement

Missed Appointment/No-Show Policy:

A missed appointment or no-show is defined as the patient failing to attend their scheduled appointment. The patient will incur a charge equivalent to 100% of the visit fee for any missed appointments or no-shows. Should an established patient violate the no-show and/or late cancellation policies twice or more within a single calendar year, Legacy Family Psychiatry reserves the right to terminate the patient-physician relationship and dismiss the patient from the practice.

Late Cancellation Policy:

A late cancellation is defined as a patient initiating an appointment cancellation within two business days of the appointment date without written approval from the physician/provider. The patient will be charged 100% of the visit fee for any unapproved late cancellations. If an established patient violates the no-show and/or late cancellation policies twice or more within one calendar year, Legacy Family Psychiatry reserves the right to terminate the patient-physician relationship and dismiss the patient from the practice.

Patient Dismissal Policy:

Legacy Family Psychiatry reserves the right to end the physician-patient relationship at any time and dismiss patients from the practice. While giving a reason for dismissal is optional, frequent no-shows, late cancellations, and unprofessional conduct are common causes for discharge. If you need ongoing care, it’s important to find another physician as soon as possible. The dismissal letter will include suggestions for other psychiatric offices, and with written authorization and a signed release form, your medical records will be sent to your new provider.

Medication Refill Policy:

Patients or their guardians are responsible for making sure they have enough medication on hand. Legacy Family Psychiatry will supply enough to last until the next appointment. If any dosage changes are planned, updated prescriptions will be given at that time. Medication refills aren’t urgent and will only be handled during regular business hours, and no refills will be given if appointments are missed.

Controlled Substance Policy:

Controlled prescription medications must follow federal and state laws, along with standard prescribing practices. Misusing these medications can cause side effects, addiction, or overdose, potentially leading to death. It’s important for patients or their guardians to fully understand the rules for using these medications. Anyone receiving controlled substances must sign a form confirming they understand our prescribing policy. Patients are expected to attend follow-up appointments as directed. No extra or early refills will be given for any reason, including lost medications. Legacy Family Psychiatry does not prescribe Xanax/alprazolam due to its highly addictive nature, and other controlled medications are prescribed based on individual needs. We also do not prescribe opioids or pain medications.

Disability, FMLA, and Emotional Support Animal Request Policy:

Completing disability or FMLA paperwork takes considerable time from our physicians and staff, so an additional fee will apply. The physician will use their clinical judgment to decide if the patient meets the criteria for disability. Please note that Legacy Family Psychiatry does not provide letters for emotional support animals..
